POLICY AND PROCEDURE:

• I reserve the right to decline accepting any new patient and to suspend or terminate treater-patient relationship;

• Patients must have underwear on at all times;

• No needles or any other treatment will be performed in or around patient’s genital area;

• Payment must be made at the time the treatment is rendered;

• I do not accept credit cards or out of state checks;

• Appointments are made on a first come first served basis;

• There will be no refund to patients who did not receive the treatment for failure to comply with the policy and procedure.

• You are liable for the full price of your appointment if you fail to cancel a scheduled appointment in at least 24 hours.
